Guidelines & Trials

Landing page for wiki/guidelines-trials/ — landmark RCTs and society guidelines (NCCN, ASCRS, ESMO, etc.).

  • SCANDIV Trial — landmark RCT (Sweden/Norway) comparing laparoscopic lavage vs. primary resection for perforated diverticulitis. 5-year report ingested; 90-day/1-year reports still to add.
  • Ladies Trial — landmark RCT (Netherlands/Belgium/Italy), two arms both ingested: LOLA (laparoscopic lavage vs. sigmoidectomy, stopped early for safety) and DIVA (Hartmann’s vs. primary anastomosis for Hinchey III/IV, primary anastomosis strongly favored).
  • ACG 2025 - Ulcerative Colitis Guideline — 2025 ACG society guideline: diagnosis/monitoring, full advanced-therapy roster for moderate-severe UC, and an acute severe UC (ASUC) management algorithm. First primary-guideline source for UC medical management in this wiki.
  • AGA 2024 - Moderate-to-Severe UC Pharmacotherapy Guideline — 2024 AGA living guideline: NMA-based efficacy-bucket positioning of advanced therapies, combination therapy (UC-SUCCESS), de-escalation, step therapy, pregnancy guidance. Explicitly disagrees with the ACG guideline’s methodology on whether to rank therapies via network meta-analysis — see Ulcerative Colitis’s “Efficacy positioning” note.
  • BSG 2025 - IBD Guidelines (Adults) — 2025 British Society of Gastroenterology guideline, by far the broadest of the three UC/Crohn’s guidelines ingested: covers both diseases plus extensive cross-cutting content (TDM, pregnancy, PSC-IBD, perioperative management, spondyloarthropathy). Closes the wiki’s biggest IBD gap — first primary-guideline source for Crohn’s medical/surgical management (previously textbook-only). See Crohn Disease’s new “Advanced therapy, monitoring, and post-surgical management” section.
